Forms

This page sets out the eleven categories that make up the framework for The Criminal Procedure Rules and sets out the forms to be used in conjunction with them.

All the current Criminal Procedure Rules foms for use by the public appear on this page, in PDF format. Since March 2011, we have been adding Word versions of the forms to this page.

Forms for use in connection with the Criminal Procedure Rules may be amended or withdrawn from time to time, or new forms added, under the authority of the Lord Chief Justice. See paragraph I.14 of the Consolidated Criminal Practice Direction, as substituted by Amendment No. 26 to the Practice Direction reported in the Weekly Law Reports at [2012] 1 WLR 778. Most of the forms are listed in Annex D.  As they are to be used in any case going to trial, the Case Management forms, for use in connection with Part 3 of The Criminal Procedure Rules, are listed separately in Annex E.
